Please make you have completed and submitted the following documents by Friday 20th May:- Copy of traveler's passport
- International Travel Document
- Notarized letter authorizing travel by minor w/o parent
- CMT Model Release Form
- CMT Player Welfare Form
Documents should be emailed to James Peterson as soon as possible.
Important Notes
- All families will need to complete a notarized letter authorizing travel for your child when accompanied by an adult who is not a family member. The notarized letter must be submitted to Seacoast United prior to departure and presented to government authorities if requested.
- The traveler's passport cannot expire within SIX month's of the expected travel period. If the traveler's passport does expire under this grace period, you will need to renew your passport as soon as possible.
-On the Authorization Travel by Minor form, please put down 'James Peterson' as the intended Seacoast United Coach.
